What happens when hospice care can't help with stage 4 melanoma?

My name is Richard, I have stage 4 Metastatic Melanoma. Went through Immunotherapycancer sucks. for 18 months until I couldn't do it anymore. After they told me there was nothing else they could do for me and put me under Hospice care. I went a year under their care until they couldn't kill me either and sent me to a Primary care doctor to find out why I'm still kicking. 2 weeks ago I went down with Congestive Heart Failure due to the combination of Opdivo and Yervoy pharmaceuticals.

I'm currently dealing with several challenging symptoms including pain, nausea, depression, insomnia, and shortness of breath. My medication list includes Oxycodone and Norco (hydrocodone plus acetaminophen) for pain, Ondansetron for nausea, Furosemide for fluid buildup, plus Iron and Vitamin D3 supplements. Managing all these symptoms together feels overwhelming sometimes, and I know I need to stay in close contact with my oncology team about adjustments.
