7 days ago**Attention** I refuse to be labeled as a snitch, who am I snitching on?! A few people at Veterans Affairs, lynn.horwatt@va.gov, lauren.castillo2@va.gov, jasonn.randolph@va.gov, & Justin.Wipple@va.gov, do not be a crash dummy for the hospital, military, police or the VA. If you're civil rights gets violated, report it! They have a long history of mistreating minorities. Look up Tuskegee Airmen. Why do I have discharge papers saying that I am a 90-year-old White male on a feeding tube and other false information. Look at https://www.youtube.com/@hackingcowards-y4b In 2024, I believe communications involving VR&E personnel and law-enforcement agencies may have contributed to law-enforcement actions directed toward me, including a response at my residence. I request preservation and review of all records sufficient to identify the existence, timing, participants, content, and purpose of any such communications. I further request that investigators determine whether any information communicated was accurate, complete, supported by available records, and free from material omissions, and whether any such communications influenced subsequent law-enforcement decisions or actions affecting me. privilege escalations hacking that is highly illegal. VR&E Misconduct Documentation VR&E Timeline Summary • Oct 2020: Accepted into VR&E for DeVry cybersecurity program. • Nov 2021: Finally allowed to start due to VR&E delays under Lynn Horwatt. • Feb 25, 2023: Graduated with 3.97 GPA, dean’s list. • Aug 2023: Removed from VR&E despite completing program and despite ongoing communication. • Aug 2023: You filed daily complaints with White House VA Hotline. • Lauren Castillo (Reno VREO) repeatedly closed your complaints falsely claiming “We already discussed this.” • Justin Whipple removed you from VR&E claiming you “stopped communication,” contradicting Castillo’s claim. • 2024: VR&E allegedly lied to law enforcement → federal charges filed. • Aug 2024: Your apartment was SWATed. • Early 2025: Charges dropped at jury selection; they claimed they “didn’t know how disabled” you were — despite VR&E having full documentation. Illegal hacking, please help. https://www.youtube.com/@hackingcowards-y4b This is a pattern of administrative abuse, negligence, and retaliation. https://www.youtube.com/@hackingcowards-y4b Please investigate lynn.horwatt@va.gov lauren.castillo@va.gov Justin.Wipple@va.gov Jasonn.randolph@va.gov L https://www.youtube.com/@hackingcowards-y4b PUBLIC / REDACTED AFFIDAVIT OF ALEX EVANS I, Alex Evans, being duly sworn, hereby declare under penalty of perjury that the following statements are true and correct to the best of my knowledge, information, and belief. 1. Personal Information Name: Alex Evans Date of Birth: [REDACTED] Last Four Digits of SSN: [REDACTED] Current Location: [REDACTED] Email Address: [REDACTED] Telephone Number: [REDACTED] Personal identifying and contact information has been removed from this public version. An unredacted version may be provided directly to authorized investigators, attorneys, courts, governmental agencies, or other appropriate officials. 2. Purpose of Affidavit This affidavit is submitted to document facts, events, records, communications, and circumstances that I believe warrant independent investigation and review. The matters described herein include allegations involving disability discrimination, racial discrimination, retaliation, denial of accommodations, unsafe discharge practices, potential civil-rights violations, administrative misconduct, medical negligence, and alleged cyber-related harassment involving personnel associated with the Department of Veterans Affairs (VA), the Salt Lake City VA Health Care System, University of Utah Hospital, VA Vocational Rehabilitation and Employment (VR&E), and other involved entities. I respectfully request a thorough, impartial, and evidence-based review of all relevant records, communications, decisions, and actions to determine whether any violations of law, policy, professional standards, patient-safety obligations, disability-access requirements, or civil-rights protections occurred. I further request preservation of all potentially relevant records pending completion of any review or investigation. 3. Unsafe Hospital Discharge – University of Utah Hospital On or about July 9, 2026, I was discharged from University of Utah Hospital under circumstances that I believe were unsafe and inconsistent with my medical and disability-related needs. Hospital security personnel transported me in a wheelchair to a public transit location despite circumstances that included: • My inability to independently self-propel a wheelchair; • My belief that an appropriate discharge plan had not been established; • My belief that my medical condition remained unstable; • The absence of transportation or assistance necessary to safely reach an appropriate destination; and • The failure to provide accommodations I believe were reasonably necessary because of my disabilities. I request an investigation to determine whether the discharge complied with applicable federal and state requirements, hospital policies, patient-safety obligations, discharge-planning requirements, disability-access requirements, and other legal or regulatory standards. I further request preservation and review of all relevant medical records, discharge documentation, transportation records, security records, witness statements, surveillance recordings, and other evidence related to this event. Video Evidence: A video documenting relevant events has been provided separately and should be preserved and reviewed. 4. Medical Placement – Monument Health Stonecreek, Bountiful, Utah I was placed at Monument Health Stonecreek, a rehabilitation facility, under circumstances that I believe warrant independent review. I question whether this placement was medically appropriate and whether adequate clinical justification existed for the placement decision. Specifically, I believe the placement may have been: • Medically inappropriate; • Outside my appropriate service area; • Insufficiently supported by clinical findings; and • Harmful to my health, safety, and well-being. I request an independent review of all medical records, referral records, placement decisions, clinical evaluations, transportation decisions, administrative records, and communications regarding this placement. I further request investigation into whether disability discrimination, medical negligence, administrative negligence, improper placement procedures, or other violations occurred. 5. VA VR&E Conduct and Retaliation Concerns (2020–2024) I was accepted into the VA Vocational Rehabilitation and Employment (VR&E) program in October 2020. I believe administrative delays prevented me from beginning my program until November 2021. I successfully completed my educational program and graduated in February 2023 with a 3.97 GPA. In August 2023, I was removed from VR&E participation. I dispute the reasons provided for this action and contend that the stated justifications should be compared against the underlying records and communications. I submitted numerous complaints through official channels. I believe several complaints were closed using explanations that were inaccurate, incomplete, unsupported by available evidence, or inconsistent with other records. Individuals whose actions I request investigators review include: • Lynn Horwatt; • Lauren Castillo; • Justin Whipple; and • Any other personnel identified through investigation. I contend that significant factual disputes exist regarding: • The administration of my VR&E case; • Adverse decisions affecting my participation; • The handling and closure of complaints; • The accuracy of statements regarding my communications with VR&E personnel; • Whether retaliation or discrimination influenced decision-making; and • Compliance with applicable laws, regulations, policies, and disability-related obligations. I request review of all relevant records, including VR&E case-management records, counselor notes, supervisory communications, complaint records, call logs, emails, internal messaging records, and other administrative records relating to my case. 6. Communications with Law Enforcement In 2024, I believe communications involving VR&E personnel and law-enforcement agencies may have contributed to subsequent law-enforcement actions directed toward me. I request preservation and review of all records sufficient to identify the existence, timing, participants, content, and purpose of any such communications. I request preservation and review of all communications, referrals, reports, emails, notes, memoranda, telephone records, and related documentation reflecting any contact between VA personnel and law-enforcement agencies concerning me. I request that investigators determine: • Whether such communications occurred; • The identity of individuals participating in those communications; • The information communicated; • Whether the information communicated was accurate, complete, and supported by available records; • Whether material omissions occurred; and • Whether such communications influenced subsequent law-enforcement actions. I further request review of records concerning criminal proceedings that were subsequently dismissed or otherwise terminated, including whether individuals involved in decision-making had access to disability-related information contained within relevant records. 7. Ongoing Retaliation and Cyber-Related Allegations I continue to report incidents that I believe may involve retaliation, interference, suppression of complaints, unauthorized access to electronic accounts or communications, or other improper conduct. I have publicly documented some of these concerns through publicly available materials, including information published through my YouTube channel and other records maintained in my possession. I request independent review of available evidence and any relevant forensic, administrative, electronic, or investigative records to determine whether unlawful or improper conduct occurred. I further contend that adverse actions appeared to occur after I submitted complaints, requested accommodations, challenged adverse decisions, sought assistance from oversight bodies, and pursued review through official channels. I request investigators determine whether any actions affecting me were motivated by, influenced by, or related to protected activities, disability status, accommodation requests, or complaints submitted to governmental authorities. 8. Material Factual Disputes Requiring Investigation I contend that significant factual disputes exist concerning: • The reasons for actions taken regarding my VR&E participation; • The handling and closure of complaints; • Communications between agency personnel and outside entities; • The extent to which disability documentation was available to decision-makers; • The circumstances surrounding law-enforcement actions affecting me; • Medical decisions affecting my care; and • Administrative decisions described within this affidavit. Because these issues cannot be resolved solely through personal statements, I respectfully request that investigators review the underlying records, communications, and documentary evidence necessary to determine the facts. 9. Evidence Preservation Request Because many of the events described herein involve electronic communications, governmental records, medical records, and digital evidence, I specifically request preservation of: • Emails and attachments; • Internal messaging records; • Text messages; • Mobile-device records; • Telephone records and call logs; • Case-management records; • Complaint-tracking records; • Audit logs; • Medical records; • Transportation records; • Security records; • Surveillance recordings; • Electronic metadata; and • Any other potentially relevant electronically stored information. This request is made to prevent loss, deletion, alteration, or destruction of potentially relevant evidence. 10. Requested Actions I respectfully request: • Assignment of a formal case number, complaint number, or reference number; • Immediate preservation of relevant records and evidence; • Investigation into the circumstances of my hospital discharge; • Review of compliance with disability-access, discharge-planning, and patient-safety requirements; • Review of my placement at Monument Health Stone creek • Investigation into the administration of my VR&E case; • Review of communications involving law enforcement; • Review of disability-related records and decision-making processes; • Investigation of allegations of discrimination, retaliation, administrative misconduct, and other matters described herein; • Production or disclosure, to the extent permitted by law, of records relied upon in making decisions affecting my rights, benefits, medical care, placement, or interactions with law-enforcement agencies; and • Protection against further retaliation arising from the reporting of these concerns. 11. Declaration Under Penalty of Perjury I submit this affidavit in good faith and request that the matters described herein receive independent review by individuals who were not personally involved in the events at issue. I respectfully request written confirmation that this affidavit has been received, preserved, and referred to the appropriate investigative, administrative, oversight, or legal authority for review. I further request notification of any assigned case number, complaint number, investigative reference number, or other tracking identifier associated with this submission. I declare under penalty of perjury under the laws of the United States that the foregoing is true and correct to the best of my knowledge, information, and belief. Executed on: __________________________ Location: _____________________________ Signature: ____________________________ Printed Name: Alex Evans PUBLIC REDACTION NOTICE This public version intentionally omits personally identifying and sensitive information, including date of birth, Social Security information, telephone number, email address, and current medical location. The unredacted affidavit may be provided directly to authorized governmental investigators, attorneys, courts, governmental agencies, or other appropriate officials when necessary to verify identity, evaluate evidence, and investigate the matters described herein.
