Can lumpectomy margins affect chemo for stage 1 breast cancer?

Hi Christa. I traveled this same path almost exactly 1 yr ago. IDC stage 1 grade 2, ER/PR+ HER2- but I also was diagnosed with metastasis to lymph node from the biopsies. I had lumpectomy + 6 lymph nodes removed on Sep 11 2025. All margins were clear. Then I had Oncotype Test done to determine my cancer recurrence score. Recurrence score was low , so chemo not needed. Radiation therapy x 20 sessions with boost. Been taking Letrozole for 9 months. I am 69 yrs old and experiencing menopause for the 2nd time. I understand the nerves, the fear, the anxiety. Mary the Letrozole is that a hormone blocker? That's what I'm afraid of being on the hormones blocker. People said they get pain, tired. I have 0 tolerance of pain.

Yes, Letrozole is hormone blocker. I have had a few side effects but nothing I can’t manage. Your oncologist has 3 to choose from.

Christa, how old are you? The younger you are (further away from menopause), the more the hormone blockers will impact you in terms of side effects. Post menopausal women can have a different reaction than a 30 year old. (For example). I am on letrozole. 58 yrs old. Some joint pain and minor hot flashes.

Your story sounds like mine. My breast cancer journey includes IDC, Stage 1, grade 2, 8mm tumor. Lumpectomy surgery was August 7th. 3 lymph nodes were clear. I was told no chemo should be necessary. My team assumed radiation 3-5 weeks and hormone receptor for 5 yrs. I am 61. Tumor was at 10:00 on right breast with a tiny, tiny dot of a tumor at 11:00. Both were removed during surgery. Then the genomics test was done. It came back at 35, which is high (anything over 26 is high) which stated chemo was needed. My oncologist was confused so my info was sent to Mayo Clinic. Mayo would have most likely started over in assessing my situation, and a possible surgery for the tiny tumor they didn’t see. I still would not be a candidate for chemo. My oncologist and I agreed that I was not making a costly trip to Mayo Clinic only to not need chemo. So now, I am waiting for first date for 21 sessions of radiation therapy. I am starting to feel better but I am very frustrated with bras and symmetry. My right breast had the tumors. my left breast was always a bit larger but now even more so. And my right breast from what I understand may become slightly smaller through radiation. I heard somewhere that in our state (ND), there is a law that allows reconstructive surgery on the larger breast for symmetry. I’ve asked couple of medical professionals and I’m not hearing what I need to hear. The last one said to ask my primary care dr about it and use my scoliosis and back pain as a reason. I can’t believe that no one helps us to feel like our old selves, or as much as possible. I realize breast cancer changes our lives but I’m frustrated. I’m living in sports bras and tired of them already. I’m 6 weeks post surgery. I hope you come through with positive results along your journey. Wishing you all the best going forward. 💖🎀

Hi Amy. Radiation not bad. Keep everything clean and moisturized. The worst part was the radiation to my axillary. Every bra I had rubbed on the axillary scar but it was more uncomfortable to go without a bra. I started wearing JoySpun sleep tank tops with built in minimal support from Walmart. Luckily it was winter and I could layer those under button up tops for work( work is casual dress everyday).

Mary I had lumpectomy on 9-10 and then had fluid in my left breast which the nurse practitioner took 98cc of fluid out. I was told to wear a bra for 2 more weeks. I'm waiting for the oncotype to come back. I just message my Dr yesterday about the result.

I am on letrozole. It isn’t bad. Some minor hot flashes and some joint pain, but my tumor went from 35% actively reproducing cancer cells to just 13% after 6 weeks. It was definitely worth it. They got the entire tumor including margins at surgery. Yay. My oncotype was 26. If your dr is recommending estrogen blockers, I’d give them a try! There are several and you can find one that works for you! Good luck!
