Why am I getting photon radiation instead of proton for left breast cancer?

I discussed my assessment/ plan for type of radiation I will be getting For HER2positive cancer well I went in for a stimulation and to have marker put on body ,And have CT scan,I was getting a little confused because they were placing makers on my Right breast but my cancer is in Left breast,I asked why are you putting markers on Right Breast under my arm my cancer is in left Breast and then had markers put under left arm, on left breast in outer breast at near the center of chest and 2 marker on both sides,They said they were put in those places to be sure radiation beams will be in right direction, all very confusing to me .I thought i would get proton radiation because my cancer is in the left breast . I was reading for left Breast Cancer because Heart is in this area they try to use proton instead of photon and I look at plan its photon not proton, and i thought my radiation oncologist would at least go over my Ct scan and show me areas of cancer and lymph nodes that they will be getting radiation, because my surgeon got out the core of tumor and all margins were clear and all six pieces of tissue she took out around my core tumor was benign I am stage 1 with some lymph vascular invasive but I just don't want to be getting aggressive Radiation if i dont need it because as I told my oncologist radiologist I do see a cardiologist and want to protect my heart and lung area as much as possible.I am suppose to have five weeks of external beam radiation with MV6 photon also sounds pretty aggressive to me for a 1.4 mm tumor that was taken out and clear margins and all surrounding tissue my surgeon took out was benign. What are you all thought on this that have already had Radiation on left side for HERS positive cancer stage 1. I try to advocate for myself because there is a saying quality or quantity of life.I will do the radiation treatments just don't want to be over zapped and get bad burns or something that would cause my heart damage.

It's completely understandable to feel confused about radiation planning and want to protect your heart and lungs. The choice between photon and proton radiation depends on many factors that your radiation oncologist considers, including your specific anatomy, tumor location, and what's available at your treatment center. Asking questions about your treatment plan shows great self-advocacy - consider requesting a detailed discussion with your radiation team about why photon therapy was chosen for your case and what heart-sparing techniques they'll use during your treatment.

I totally agree with asking for that detailed discussion - and you might also want to ask specifically about the heart-sparing techniques they plan to use, like breath-hold treatments, which can really help protect your heart during left-sided radiation.

Thank you for responding because i live in a small city my radiologist oncologist just let me know we don't have proton radiation in our community, but he reassured me that Breath Holding technique will bemysef.

Fair questions which I’m asking myself. This recent study showed a minimal difference in long term outcomes of women who had radiation and those that did not. https://www.nejm.org/doi/full/10.1056/NEJMoa2412225

I didn't have a mastectomy my surgeon opted for lumpetocomy because of the size of the tumor and it was I guess also because of my health . I didn't have to stay in surgery so long and she was able to get it done within a week of diagnosis.

I talked to my radiation oncologist today because I had some questions as to how he could make assessments/plan on November 5 and he didn't actually do my CT Scan until yesterday November 13,How could he write up plan unless he was going by information my Oncologist Surgeon had put in her report and I know they all work together but my local Cancer Center does not even have Breast Surgeon just General Surgeon That's why i went about 1 hour awake to get a Lady Surgeon who specializes in breast cancer and Surgery .He had some expectations of course and I want to trust him because I will be getting five weeks of radiation if my body does not reject it or I get some low lab work and I will discontinue the radiation at that point ,I told him i do see a cardiologist for preventive measures and he said we will teach you about breath hold technique.

I had clear margins and minimal lymph node involvement. I am not doing radiation. My husband thinks I am crazy for not doing it. I was considering possibly trying it but only wanted lymph nodes zapped. No need for breast. I was told that protocol was all or nothing. If I didn’t agree then insurance would not pay for treatment. I would think they would want to pay for less treatment not more. Anyway, back to no way no how. My cancer type, treatment, and age indicate similar recurrence and survival with or without.

@Mary W Ask them if they use VMAT radiation - that’s what I had with breath holding and was very helpful.

I looked at Plan for radiation he is using IMRT/VMAT and breath holding techniques.

I had a 1.3 cm tumor, no node involvement, ER+/PR+/Her2-, low risk and clear margin 18 years ago. I had full radiation. The tumor came back this year to my bones. I am glad I had the radiation or else I would be kicking myself that I had not done enough to help myself. Anyway, I had a delightful 18 years which could have been less without the radiation. I’m 72 now so can deal with the metastatic issues better. But really the radiation confusion at your facility is somewhat disturbing.

I spoke with my radiologist oncologist and after talking to him I got some answers. The Cancer Center they don't have proton radiation in my area , and I am doing the IMRT/ VMAT radiation and he told me about the breath holding that will also be done to help with less damage to nearby organs.I advocate for myself that's why I had so many questions before I start radiation . Did you have proton or photon radiation 18 years ago .I think I am Ok and will start radiation Monday December 1 .How many weeks did you do was it five weeks ? When you say full radiation .I am HER2 positive.

I don’t remember what type of radiation but I do remember it was 6 weeks long. They gave me a cream which I used religiously and only burned towards the end.

Only Five treatments radiation at Rush in Chicago in 2021. Her 2 plus lumpectomy with 4 clear lymph nodes. All left side near nipple. If I could breathe on my stomach I could have protected heart. Too congested and my middle in the way so had to lay on back. No breath hold learned. Clear mammo 2025 but echo showed slight reduction of heart function I think "effusion rate" I think. Cardio said no biggie. I did hylecta shots 4 months too.

I had five Radiation treatments( one week) and I did the breath Hold technique which is hard at my age but i did for all five days and then I started having eye problems,So many floaters and flashing light in peherrially vision went to my eye doctor I had some blood behind eye and a hole in retina and he said something about detached retina I am going to see a specialist on Wednesday .My oncologist called me because I had to take a break because I do remember I looked at the green laser beams that line up the radiation machine No one ever said anything about protecting eyes with something.

Get a new DR! I was treated by UCLA Health. My cancer returned n was found in my spine… 3 places. My Oncologist from UCLA oncology in Thousand Oaks… California… Told me that my cancer had returned and I would sadly have to have hemotherapy every three weeks… Five days for that week… Five hours each day of chemotherapy. They told me I should add a port since my veins would not be able to sustain this long-term protocol for the rest of my life! I went to City Of Hope. I got a second opinion… I have focused on a strict chemo drug pill form… Cutting your dosage to avoid side effects can mess up your recovery and sustainability of the drug keeping cancer at bay. Instead of checking with my medication… Which I had reduced to 1/3 of what was required due to the side effects my doctor and PA guided me to do that… That turned out not to be strong enough and I was told I needed to do chemotherapy for the rest of my life! City Of Hope saved me… She thought the diagnosis for chemotherapy every three weeks was bogus! I just had to adjust my medication and deal with the side effects… I am doing Fine. Cancer is not being seen in my spine anymore… Going in for a PET scan every six months keeps you informed! Don’t let your Dr be the only one to decide always get second or third opinions. UCLA Health is all about $$$$$$
