How to apply for disability with carpal tunnel and blood clots?

Good afternoon, Today I actually got through to Social Security. I now have an appointment to get help signing up for disability. My long-term disability through work told me I have to sign up through Social Security. I can't type with accuracy as I have severe carpal tunnel. I will need surgery but I am not sure when. It scares me to have surgery because I have a chronic blood clot in my thigh and am on Eliquis. I am sure they will come up with a plan. So this is all good news but a bit scary.
