Does anybody have a Gleason 9 and has Hormone Therapy with external Radiation.

PSA 11, Gleason 9 (4+5), 3 yrs. Lupron concurrent with 2 yrs Zytiga, and 45 weeks IMRT radiation at 81 grays total. Started 2/19. From 2 months after start PSA has been <.1. Now at the 6 year mark. What questions can I answer?

Thankyou for getting back to me.I just started Hormone Therapy 2 different pills.They want me to take for 18 months.After 8 weeks they will do Radiation for 8 weeks.Mine spread to lymph nodes.Did yours spread out side of Prostate.

PSA 37, Gleason 9 , 3 years LUPRON and 2 1/2 years Zytiga . Stage 4a, Yes mine had spread to lymph nodes. Radiation therapy forty four times. Not much I don’t know about this disease!!! And there are no experts as they like to call themselves!!! I just had and second biopsy taken of my prostate ( Unheard of ) I forced them to do it !! They complied !! Prostate was clear of cancer , but the lymph nodes are the major concern . If just one cancer cell escaped , well it will show in a year or so in another place !!!!!!

Gerard, I’m a T3b, N1, M0, which means both loves of the prostate, extra capsular extension, 1 lymph node, but no distant metastasis, also called a Stage 4a. Treatment is “palliative,” not curative. I’m six years post diagnosis, and as of May, PSA is still <.1, considered undetectable. I was told that another biopsy would be considered on recurrence so that DNA testing could be done on cancer cells. Because they evolve, a PARP inhibitor drug might be indicated, which could be combined with Pluvicto. We’ll cross that bridge, perhaps….

TD H are u still on abiraterone/zytiga ? I’m still taking that med past the 2 year period !! And I’m unsure if I should stay on it until it stops working or to come off and risk spread in a year or so!! With a lymph node involved we are just one step away from the disease spreading and to be honest , just one step from a lot worse !! With Stage 4a it is possible to be cured of the disease but unfortunately the odds are not in our favor !!

Agree on odds. I started Lupron early in 2019, and continued that for 3 years. A month or so after starting Lupron, I began Zytiga and prednisone, and continued those for 2 years. 45 weeks of radiation were begun shortly after starting Zytiga. I’ve been <.1 after starting Zytiga, and remain undetectable at ~6 1/2 years from diagnosis. The treatment plan followed the Stampede trial, which you may want to reference. I’ve also looked at Kaplan Meier curves for hi dose radiation treatment of Gleason 9-10 PCa. It provides an estimated prognosis for biochemical failure and overall survival. FYI, I was just shy of 71 at diagnosis. The plan on recurrence, as of now, is Lupron til biochemical failure, a PSMA scan, then Pluvicto, unless other new therapies become available. Surgery was never presented as an option because of certain lymph node involvement and possibly seminal vesicles. I didn’t seek a second opinion.

It is possible that Zytiga may be added with Lupron on recurrence, or possibly enzalutimide, though these weren’t discussed.

Yes !!! If it worked once it can be taken again. It may/ may not work , but I personally would take it again and find out !! I may never come off of it , I’m not sure !!

I just started hormone therapy. Main side effect so far is hot flashes. How long do the hot flashes last?

I was on Lupron for 3 years. Though not terribly bothersome, intermittent and infrequent hot flashes ( more surges when I was slightly overheated anyway) persisted for the duration of treatment and then some.

My initial evaluation was Gleason 9, 2x8, 4x7. Went for 2nd opinion, "all 7s". Been on Orgovyx 5 months, Zytiga 2 months, PSA 11 ^38 down to 7at 2.5 months. Then, down to 1.5 to 0.38. Prostate has shrunk 65%. Radiation will start July, 28 treatments, then hopefully done after some time on ADT! God needs to be #1 in Everyone's Protocol. Glad to advise/help anyone! Jesus has paid for Our Healing!

Rick, I received 81 Gys of radiation in 45 fractions. That’s 45 days at 1.8 centigrade per day. Radiation started about 2 months after ADT started.

Hey guys...don't listen to the naysayers. Stage 4 G-9 Grade 5 CAN be cured now. Lots of new stuff....ask your Dr. Lots of questions and don't believe the....I'm gonna die stuff. Up to 70% can live a full lifespan.

OK, I'm hearing lots of mixes but mostly Lupron, then add Zytiga. Then, radiation. After 3rd opinion(7 doctors), I picked 2 I felt most comfortable. First guy tried to create panic, didn't buy it & moved on. Finally started Orgovyx Feb., added Zytiga May. Kinda pushed 2nd mri after 2.5 months on ADT. 2nd mri shows prostate at 49 mL, down from 85 mL. Doc refuses to acknowledge it matters? Started 28 VMAT fractions on last Wed. Previous PSA finally down to 0.17 pre-radiation. Expecting good psa next Tuesday! I learned way more than I ever wanted to know but main point I believe about Drs is they want to do whatever their specialty is, not that they're doing anything wrong, they just get into their groove & usually think they'll help you. Be your own advocate unless your a person that can just rely on them, I can't! Lastly, but Most Importantly, Lean on God, He's Faithful & Jesus Paid for it!

Rick, you may want to read about the Stampede Trial. It’s the treatment paradigm my oncologist followed starting in 2019. It seems to have been more widely accepted. I’m at 61/2 years with <.1 PSA. Labs in 3 days. Praying. Meanwhile, feeling well and counting blessings.
