5 days agoI've been looking into treatment options for my prostate cancer and learned about focal therapies like HIFU and cryoablation. These treatments target only the cancerous part of the prostate to try to preserve normal function. My doctor explained that these are considered experimental and aren't typically recommended as first-line treatment since there aren't long-term studies showing they work as well as proven treatments like surgery, radiation, or active surveillance. I'm curious to hear from others who may have faced similar decisions about treatment approaches. • Has anyone here considered or tried focal therapy? What was your experience with the decision-making process? • For those who chose standard treatments, what factors helped you decide between surgery, radiation, or active surveillance?

3 days agoI faced a similar decision and a few factors unique to my situation led me to the path I chose. It wasn’t easy but I was glad to have good doctors and support I was able to connect with that had bee down the path I chose. To start, I had a positive biopsy with a Gleason 6. This was called for after my PSA went to 4.12. Before that my PSA was increasing but always below 4. I also had BPH for years as did my Dad and my brother. I was exploring TURP for the BPH when my last PSA before my scheduled TURP. The PSA scared me and when my urologist told me that after TURp, radiation would be my next option, I cancelled that surgery and chose a new doctor. I was told by the new doctor that prostate removal would still be an option after TURP, so I learned how important second opinions are. I also did a decipher test that had a .61 result - hi risk of dropping out of active surveillance which was a key decision factor for me. I explored HIFU for the cancer and a separate Aquablation for the BPH. However, ultrasounds and MRIs showed cancer on both sides of the prostate, so my doctor said HIFU wasn’t an option for me. At the end of all this his recommendation was active surveillance. My wife and I discussed it in depth and we chose radical prostatectomy. After all the twists and turns of the diagnosis and possible procedures we thought this gave the best chances and the possibility of addressing both issues. Thankfully, the surgery went well. Post procedure pathology showed I was a Gleason 7 and thankfully no surrounding spread has shown up. My recovery was gone well, and while it’s slow with occasional plateaus, I’m making steady progress. I’m 15 months out and my PSA has been non-detectable. Incontinence is lessened and I’m almost there. ED is still an issue and I’ve been told it can take 2 years, but there are no guarantees. We pray for no recurrence and a complete recovery. Mainly I try to live every day and hope for the best. I exercise and try to eat right to give myself the best chances. I now sleep though the night which compared to getting up 2x a night, is simply life changing. Overall I feel we made the best decision for the cards we were dealt. We’re trying to look forward and be thankful for the good fortune we’ve had, and pray it continues. No one ever wanted to be a part of this club. But given it’s the club I’m in, I appreciate knowing I’m not alone and happy to help others as there have been so many to help me, starting with my wife. 3 days agoHi l had hifu in Florida, l won't say the name of the doctor but l still had cancer after hifu, l ended up having a prostatectomy at Mayo, my psa has gone up every three months, radiation is my next option. I heard Mayo does hifu now, if l were you and you're considering hifu l would consult with Mayo or MD Anderson, good luck to you
