Why won't my doctors perform a mastectomy for metastatic breast cancer?

Hi Everyone, Has anyone had an oncologist and or breast surgeon refuse to perform a mastectomy? I am MTNBC, diagnosed almost 1 yr ago. Chemo wasn't successful so now I'm on Trodelvy (ADC) and Fulphila. I always wanted a mastectomy as I see my implants as the root of the problem and the breast as food/fuel. They said the cancer is already spread so no sense operating. Maybe so, but why can't I get tumors removed? Thanks for your opinion.
