2 months agoDiagnosed with pT2 MIBC in Nov. Based on results from Keynote Trial, FDA had just approved Padcev and immune therapy Keytruda for MIBC plus hearing loss. Keytruda caused issues with thyroid and elevated liver enzymes. Had to take prednisone to get them back down. Caused glucose issues for a diabetic but managed. TURBT and 6 biopsies showed pT0. Looking at bladder sparing vs cystectomy via new clinical trial using radiation plus immune therapy Keytruda. Concern about rechallenging liver with Keytruda but can stop that after radiation and move to surveillance if Keytruda becomes a problem again. Anyone with experience with the BRIGHT 2427 trial?

2 months agoThis sounds like a complex decision with important considerations around treatment options and managing previous side effects. While specific experiences with BRIGHT 2427 may be limited since it's a newer trial, connecting with others who have navigated similar treatment decisions or dealt with immunotherapy side effects could provide valuable insights. The bladder cancer community here has many knowledgeable members who understand the challenges of weighing bladder-sparing approaches against more definitive treatments, so hopefully some will share their perspectives to help inform your discussions with your medical team.
