Why do I need lumpectomy before mastectomy?

Hi Everyone! At the beginning of February I was diagnosed with stage 1 TNBC, but grade 3 (fast spreading), so my Oncologist recommended chemo before surgery. I did my 4 infusions of Red Devil and just last week completed my 12th infusion of Taxol. In a few weeks I am scheduled to have a lumpectomy and breast reduction. Then, after 3 months I will have a bilateral mastectomy with reconstructive surgery at the same time. At the beginning of my journey, the 2 surgeries seemed to make sense based on the Surgeon and Oncologists reasoning, but now I’m starting to question it more. Honestly I can’t remember what their explanation was (chemo brain!), but as more people ask about it, I’m wondering too. If I am having everything taken off, what is the point of a lumpectomy? Why can’t I just have the one surgery and be done? Has anyone else had a similar treatment plan to mine? I feel like mines all backwards…

I had skin sparing dbl masectomy with reconstruction and implants in '23. I was given a 5% chance of further BC. Early '26, I was diagnosed with what you have. The total opposite of the type I had in '23! Had the same as you, chemo 1st. My dr said they get better overall results if the chemo comes first. No red devil. I was in a clinical trial to see if less chemo drugs would be as effective and only 6 treatments. Nope. I have surgery next week. And I honestly think the tumor is bigger than when we started. Im scared to death in the weeks Ive waited for surgery with no treatment that its grown and terrified of spreading. I guess we'll see next week. I just hate this. I want this thing out of me.

I’m so so sorry to hear yours returned. I’m terrified of having to go through all of this again in the future, as I’m sure you were. As of a month ago, we were fairly confident that mine was gone from the Signatera testing and an MRI, but will find out for sure after my lumpectomy in 2 weeks. It’s still nerve wracking waiting for the next step when in the beginning of treatment you’re told how urgent it is, yet knowing that the wait is most likely due to insurance and scheduling issues. I kept joking with friends and family that I’d cut them off myself if I had to, but knowing that doesn’t necessarily mean it won’t be back is so scary. Thank you for sharing your story with me, and know that I will be praying for you and for positive results from your surgery next week.
