Is chemo causing liver damage even with good cancer response?

I was diagnosed with stage 4 pancreatic cancer with Mets to liver almost a year ago. I am 68 years old. I have been getting chemo every two weeks and responding very well with little side effects except neuropathy. Two months ago my chemo (FoLFIRI) was modified because of the neuropathy. Just yesterday , my chemo was modified again (they reduced a dosage of irinotecan) because my GAMMA (GGT) and ALP were trending up. Signatera test was negative and Guardant found no tumor. CT scan showed no new tumors over the last year and stable or slightly reduced tumors. I am blessed and grateful for my positive treatment response but worried about the chemos effect on my liver. What are your thoughts?

It's completely understandable to have concerns about liver function while undergoing chemotherapy, especially when lab values start trending upward. The encouraging news is that your medical team is actively monitoring these levels and adjusting your treatment accordingly - this type of careful dose modification shows they're prioritizing both your cancer response and organ health. Your oncology team would be the best source for discussing the balance between treatment benefits and potential liver effects, as they have access to your complete medical picture and can explain what these specific lab changes mean in your individual case.

I’m getting treatment every 2 weeks also and only issues are hair loss and blood levels running low. Went from 100% to 80% now. My faith with God is the biggest blessing

My blood level is run low. I've been hospitalized five times. One time was the last time with blood levels too low. That certainly zaps one's strength. I see a naturopathic oncologist who is working with my vitamins, iron, and other resources to boost my immune system. She has been a godsend. My oncologist adds the labs she requested to his labs so that I don't have to pay for them. He doesn't see the good in it yet. However, he's a western medical doctor. I understand the mentality.
