For high-risk stage I cases, surgery alone may not be sufficient. Vaginal brachytherapy is frequently added for high-risk stage I cases as a preventative measure to reduce the chance of local recurrence. At this stage, systemic treatments like endometrial cancer chemotherapy drugs are generally not part of the picture, though that calculus shifts significantly at later stages.

Fertility-sparing consideration: For a very narrow group of patients with stage IA, grade 1 disease who haven’t yet completed childbearing, progestin therapy may be discussed as an alternative to immediate surgery. This option requires close monitoring and is only appropriate for carefully selected cases.

Stage II: addressing spread to the cervical stroma

When the cancer has extended this far, a single treatment approach isn’t enough. That’s where the “sandwich method” comes in. The sandwich method sequences treatment as chemotherapy, followed by radiation, followed by more chemotherapy. Chemotherapy targets cancer cells that may have traveled beyond the pelvis, while radiation addresses the regional tumor site directly. Carboplatin and Paclitaxel (also known as Taxol) form the standard chemotherapy regimen for advanced uterine cancer, and this combination anchors both phases of the sandwich.
Managing side effects across this extended treatment course is one of the most challenging aspects of stage III care. Radiation for uterine cancer side effects, including pelvic discomfort, bowel changes, and vaginal dryness, overlap with chemotherapy-related effects like fatigue, nausea, and hair loss. Peripheral neuropathy, which is a tingling or numbness in the hands and feet, is caused by Paclitaxel and is particularly common. While the chemotherapy regimens used for stage III remain part of the picture here, stage IV care increasingly centers on your tumor’s genetic profile. Your genetic profile, including genetic markers of your cancer, is one of several factors your care team will consider when designing a personalized treatment plan for uterine cancer. Immunotherapy with pembrolizumab is now a standard option for patients whose tumors carry MSI-H or dMMR markers, either alongside chemotherapy or as a subsequent line of therapy.
For patients without MSI-H or dMMR status, targeted therapy offers another modern path. Lenvatinib, often combined with pembrolizumab, has shown meaningful results in stage IV uterine cancer patients without MSI-H or dMMR status.
Clinical trials provide access to emerging combinations and novel agents that standard protocols haven’t yet adopted. In advanced and recurrent uterine cancer, clinical trials are exploring emerging therapies that may offer additional options alongside or instead of standard chemotherapy.
